Community Residence Manager (Group Home)

ESSENTIAL DUTIES AND FUNCTIONS :

  • Directly responsible for orientation, supervising, and training of staff.
  • Ensure compliance with state rules and regulations.
  • Maintain household operating budgets and bookkeeping system.
  • Maintain appropriate client files, records, and reports.
  • Coordinate and maintain all home staff schedules.
  • Attend medical appointments for residents.
  • Ensure staff compliance with all required trainings.
  • Plan and participate in community outings with residents.

Compensation: The average compensation for Direct Support Professionals at New Concepts for Living is $60,000. You can earn more or less based on overtime.

Schedules available: 3pm-11pm, 11pm-9am (overnight), some weekend days required

ESSENTIAL DUTIES AND FUNCTIONS:

  • Assist developmentally disabled residents with daily living skills.
  • Cleaning and maintenance of community residence.
  • Assist in dispensing resident medications.
  • Provide input on all residents’ care and abilities.
  • Perform any additional tasks that are in the best interests of the residents.
  • Document all required activity notes according to NCFL policy and Medicaid guidelines.
  • Assist with behavioral programs in ensuring established for residents are adhered to.
  • Transport residents to Day Program, medical appointments, and outdoor activities.

Shift Nurse (LPN & RN)

New Concepts for Living is a community based non-profit organization committed to adults with developmental disabilities and their families.  We are growing and hiring LPNs/RNs to service our community residences.

SALARY:
LPN $35/hour
RN $45/hour

SHIFTS AVAILABLE:
8-hour Shifts

ESSENTIAL FUNCTIONS:

  • Administration of prescribed medications, treatments and therapies
  • Provide ADL care, transportation, and community outings as needed
  • Assist with the maintenance and reconciliation of medication books, house logbooks, etc. to ensure compliance with all program regulations
  • Ensure medications, medical and safety supplies are ordered and available as needed
  • Demonstrate good clinical and ethical judgement in the performance of duties while respecting the dignity and confidentiality of the individuals we serve
  • Re-position in bed non-ambulatory residents who weigh 50 pounds or more
  • Other duties as deemed necessary by Supervisory personnel

  • Graduate of an accredited School of Nursing with a current State of New Jersey Nursing License or Multi State Nursing License

Day Program Nurse Administrator

New Concepts for Living is a community based non-profit organization committed to adults with developmental disabilities and their families.

SALARY: $85,000 – $110,000/per year

HOURS: 8:00am – 4:30pm                                                              

ESSENTIAL DUTIES AND RESPONSIBILITIES (include the following but are not limited to):

  • Communicate effectively with a variety of departments and providers, both verbally and in writing
  • Work cooperatively with all professional health providers, to ensure continuity of care
  • Maintain agency’s continuous compliance to the applicable regulatory requirements
  • Oversee day to day delivery of nursing functions
  • Assist with field orientation of newly hired nursing staff members as necessary
  • Conduct and participate in IDT, ISP and clinical meetings as necessary
  • Assure accurate, current medical records are maintained according to state and federal requirements and maintain client confidentiality
  • Maintains adequate knowledge in documentation requirements to serve as a resource for day program staff
  • Act as liaison between physician, nurse, pharmacist, and agency (when applicable)
  • Be flexible in changing work environment, work well in problem solving situations
  • Develop appropriate trainings per agency needs while assuring trainings are up to date per division standards

Nurse - Residential Manager

SALARY: $110,000 – $130,000

HOURS: 40 Hours per week  

ESSENTIAL DUTIES AND RESPONSIBILITIES (include the following but are not limited to):

  • Strong leadership skills in managing nurses and staff. Communicate effectively with a variety of departments and providers, both verbally and in writing.
  • Understand the overall operations and functions of a six bedroom Community Residence Program.
  • Work cooperatively with all professional health providers, to ensure continuity of care.
  • Follow admission and discharge requirements for hospitalized individuals.
  • Consult and advocate with other medical professionals and attend care meetings as required.
  • Perform and document daily assessment of each individual and recommend course of action necessary to address health care concerns.
  • Schedule and attend medical appointments.
  • Maintain compliance with applicable regulatory requirements.
  • Assist with on-call needs of the Community Residence.
  • Conduct orientation of newly hired staff.
  • Assure current medical records are maintained in accordance with state and federal requirements.

Assistant Residential Director

SALARY: $90,000 – $120,000/per year

HOURS: 50 per week

ESSENTIAL DUTIES AND RESPONSIBILITIES (include the following but are not limited to):

  • Preview, oversee and participate in resident Individualized Service Plans (ISP’s), serving as a member of an I.D.T. and ensuring that resident needs and preferences are properly addressed, and plans are appropriately implemented, monitored, and revised as needs dictate
  • Assist in the oversight of residential staff needs including but not limited to utilization, recruitment, orientation, supervision, training, and development as needed
  • Provide assistance and support to the home and apartment managers to resolve coverage and related staffing issues
  • Assist with the processing and oversight of various forms of program documentation to ensure compliance with NCFL policy and various federal, state, and local requirements
  • Communicate any and all concerns regarding homes or staff to Director in a timely manner
  • Coordinate the provision of resident transportation
  • Serve as a liaison between managers and other departments to ensure effective communication
  • Conduct manager meetings monthly
  • Oversee home schedules to assure effective coverage and reduce overtime
  • Remain up-to-date on Division Licensing and Circular requirements and assist with or initiate activities to ensure proper compliance
  • Provide on-call support as scheduled or requested
  • Ensure compliance with all personnel requirements necessary to maintain employment

QUALIFICATIONS REQUIRED:

  • High School Diploma or equivalent
  • College degree in Social Work, Vocational Rehabilitation, Special Education or Psychology preferred
  • Five (5) years’ experience working with DD population in a managerial capacity

Assistant Clinical Director

ESSENTIAL DUTIES AND RESPONSIBILITIES (include the following but are not limited to):

  • Assist in the oversight of the clinical operations of occupational, physical therapy, and speech therapy services
  • Support Clinical Director in the lead, mentor, and develop a team of therapists and support staff
  • As instructed by the Clinical Director, ensure staff maintain high levels of professional competency through ongoing education and training
  • Implement and maintain evidence-based practices in occupational, physical therapy, and speech therapy
  • Contribute to the development, review, and update clinical policies and procedures
  • Ensure individualized treatment plans are developed and followed, incorporating client-centered goals
  • Under the direction of the Clinical Director, identify opportunities for program expansion and service improvement
  • Collaborate with other departments to enhance service delivery
  • Monitor and evaluate the quality and effectiveness of clinical services
